End of Life Care tools provide practical guidance and support to help health and social care staff implement the End of Life Care Strategy.
There are tools available to guide on all aspects of end of life care from Advance Care Planning through to caring for the dying patient. There are also tools developed which provide support and direction over the entire end of life pathway, providing a framework to facilitate the delivery of high quality end of life care.
In addition to these tools the Cumbria and Lancashire End of Life Care Network have developed the Six Steps to Success Programme in collaboration with the Greater Manchester and Cheshire Cancer Network and the Merseyside and Cheshire Cancer Network. This programme incorporates the key End of Life Care tools in a package designed to facilitate organisational change and high quality end of life care in care homes.
