The second annual facilitator conference was held in Blackpool on 4th October 2012. The conference covered important topics around health and social care and provided both regional and national updates on work which has been done and the future of end of life care.

The North West groups would like to thank all of the speakers and the delegates who contributed in making this second conference a great success.
Introduction
This is the song which opened the conference and set the scene to keeping the person at the centre of everything we do, to listen to what the person is saying and try to support the person to achieve their wishes and preferences.
It is a clear message about a persons right to autonomy.
Visual messages are a useful tool to promoting person centred care ensuring the persons voice is heard. It contains an emotive and powerful message.
