North West Regional End of Life Care Facilitators Network

Terms of Reference

 

Key Aims

• The network would aim to address the varied needs of facilitators working in different sectors through support, education and training
• The network will aim to unite facilitators across the North West and share good practice

 

Roles and Responsibilities (of steering group)

• To provide timely reports as requested by the National End of Life Programme
• The North West Regional End of Life Care Facilitators Steering Group will update the Chair and members of the North West End of Life Care Operational group on the work and progress of the regional network outcomes through reports and briefings

Roles and Responsibilities (of Members)

• Each facilitator has the responsibility to share work to the group
• Each facilitator has a role and responsibility to feedback to their organisation best practice to improve patient care
• Each facilitator has a responsibility to contribute to the annual evaluation of the effectiveness of the network
